Why Do Patients Need a House Call?
- Homebound: The patient is unable to leave their house.
- The patient is unable to move around or has significant difficulties doing so.
- Mobility aids are being used by the patient.
- The patient is unable to travel to the doctor's office due to a disability or other reasons.
- A physician must evaluate the patient's home environment and the relationship between the patient and the caregiver.
House Call Types
- Evaluation of a patient (e.g., polypharmacy, suspected neglect or self-neglect, suspected abuse, social isolation, need for family meeting, excessive health care use, recent major health changes, multiple medical problems, frailty, consideration of long-term care admission).
- Taking care of sick people at home (emergency, acute, or chronic conditions).
- Death pronouncements, palliative care, and bereavement support for terminal patients
- Services provided by a personal concierge (e.g., health promotion, disease prevention).
There is no limit to the number of visits that can be made to individuals with long-term or terminal illnesses.

Medicare Definition of Homebound
A Medicare patient must meet the following requirements in order to receive home health care services:
- The patient may require intermittent skilled nursing care; or a combination of speech, physical, and occupational therapy.
- A person who is unable to leave the house and must rely on assistive equipment like wheelchairs, canes, and walkers; they must arrange for special transportation; they must enlist the assistance of another person to help them leave the house, or have medical reasons not to leave the house.
- Be subjected to a medical treatment plan that is monitored on a regular basis by a physician or healthcare professional.
- Receive home health care services from a Medicare-approved organization.
- In addition, a person should not be disqualified from being deemed restricted to the house if they meet the following criteria:
- Participation in a state-licensed or accredited adult day care program with the purpose of receiving therapeutic, psychological, or medical therapy.
- Rare or short-term absences from home, such as going to a religious service, are considered to be just that: infrequent or brief.
