NJ DOT Physical – Here’s What to Expect

- Fill out the Driver Information and Health History parts of the Medical Examination Report form and get it signed and dated.

- Please include a letter from your treating physician if you are taking medication to address any of the above diseases, or to help you stop smoking.
Urine, eyesight, hearing and blood pressure will all be tested as part of the physical exam. To be considered medically fit, you must pass each exam according to DOT standards.
Vision Test
To read traffic signs, monitor road conditions, and more, commercial drivers require strong eyesight. Each eye must be able to see at least 20/40, with or without the aid of a corrective lens. On the eye chart, you will be required to read the characters in a specific order. Additionally, the nurse practitioner will check each eye’s peripheral or side vision. For your final test, you’ll be put through a series of tests to see if you can identify between the colors of traffic lights.
Note to drivers: If you wear eyeglasses or contact lenses, bring them along.
Hearing Test
You must be able to hear a forced whisper from at least 5 feet away, whether you use a hearing aid or not, according to the DOT. If you don’t, you’ll need to take an audiometric hearing test. This is to ensure that you will be able to hear the noises of traffic and any radio announcements. Make sure to bring any hearing aids you use to your consultation.
Blood Pressure Test
You must have a blood pressure equal to or less than 140/90 to be eligible for the 24-month certification. A shorter certificate may be issued to those whose blood pressure is higher than 140/90 but less than 180/110.
